2025年5月、国際実験血液学会(ISEH)の若手研究者委員会は、造血幹細胞(HSC)遺伝子治療をテーマとしたウェビナーを開催しました 。このレビューは、HSC遺伝子治療の専門家であるLuigi Naldini教授Mitchell Weiss教授が主導した議論の内容を要約し、その基本原理や課題、さらには最新の戦略や応用例について紹介します。
  • HSPC遺伝子治療は急速に進歩しています。レンチウイルスベクターによる遺伝子導入、Cas9ヌクレアーゼに加えて塩基エディター(BE)とプライム編集 (PE)といったCRISPR技術を利用するゲノム編集、新たに登場した生体内(in vivo)送達アプローチ、然るべき編集が進行した細胞を識別・濃縮する技術(SMArTシステム [crisp_bio 2026-06-13])の研究開発が進み、遺伝性血液疾患の治療の選択肢を拡大しています。
  • 近年の臨床試験での成功により、持続的な治療効果が得られる可能性が実証されており、現在も改良が続けられることで、安全性、有効性、および実施可能性の向上が図られています。
  • 一方で、遺伝子毒性と安全性、前処置化学療法に伴う毒性、高額なコスト・複雑な工程とロジスティックス、そして世界的な公平なアクセスといった課題が依然として残されています。
